Reveals one of sixteen ancient locations at random on your world map. It has the longest candidate table of any map in the game, which means it gives you somewhere, not the somewhere you wanted.
A treasure map for some ancient ruin. Likely full of treasure or lost technology or horrible death. The map is insanely old and hard to understand, but from what you can gather it appears to be something science or technology related. It could have Ancient Science Books, or possibly even AI Cores.
The Old Treasure Map is one of the looted map items. You cannot buy it; it only comes out of chests in ancient locations. Using it consumes the item and puts one location on your world map.
Its candidate table is the longest of any treasure map in the game, sixteen targets in total. That decides how it is used: it is a consumable for lighting up a batch of ruins over time, not a tool for finding one specific place.

The sixteen entries add up to exactly 100%, and each use reveals one of them.
One entry on that table is not a ruin: Southern Hive is a city with people living in it. A map of ancient treasure pointing at an inhabited town is not explained anywhere in the game. The Scraphouse is easier to read, being a shop built inside an ancient factory, and the wiki takes the entry as the hint it looks like: the shop's high-value stock probably came out of this same ancient stash.
When it stops working
The general rule for map items is that if you already know every location the map can reveal, you cannot use it and it is not consumed.
With sixteen candidates on the table, that state is hard to reach and an Old Treasure Map is essentially always usable. What happens when you know some of them but not all is not documented on the wiki. The only certainty is the "know them all and it stops working" rule.
Where to find one
Three places produce one every time: the Lost Library, the Lost Library Outpost and the Old Control Tower. All three also hold an Engineers Map, and the Lost Library adds a set of Ancient Military Documents.
The Tower of Abuse has one on its random loot table as well, sitting in the least likely bracket alongside the Ancient Military Documents and the Engineers Map.
Naming
The official Simplified Chinese localisation renders this item as「旧宝藏之屋」, an "old treasure house", which mistranslates map and does not match what the item does. This site uses《舊藏寶圖》in its Chinese tree. The Japanese and Russian names,「古い宝の地図」and «Старая карта сокровищ», are both correct.
