A sentence that has come apart
That quote is his entire dialogue. It is not a cipher or a puzzle — it is what a broken mind sounds like. He calls a name, "Meena", and then the line slides into self-doubt and verbal tics and never comes back out.
The game never says who Meena is, or what he did to end up here. The wiki gives three facts: he is an Outlaw, he is in Tengu's Vault, and he is worth 20,000.

Screamy is a Greenlander with a 20% chance of spawning Shek. The wiki's stat block gives him no combat bonuses at all, only a randomisation of 3.
In other words the head itself can barely fight back. The whole difficulty lies in where the head is kept.
About Tengu's Vault
Tengu's Vault sits at the south end of the rocky Skimsands, guarded by United Cities Samurai. It appears to be a military prison for the empire's worst criminals. Inside it resembles a police building: ground floor beds and training area, first floor cooking and eating, second floor the cells.
The gate guards warn you off and will attack trespassers. But if you are allied with the United Cities, they only complain verbally, which lets you walk in and see what is there.
The same cells hold Luquin, Psycho, Espher, Sinklyde, Yabuta of the Sands, Hex, a Dust Bandit, and three ordinary Prisoners.
